Application Form

To apply for the Leightons Corporate Eyecare scheme, simply contact your local Leightons branch and they will be happy to go through any questions you may have, whether it's about the corporate eye test or payment for the service. They can then send you a Corporate Eyecare introduction pack that will include an application form that you can fill in and send back to your chosen branch.

Alternatively, click on the pdf below and print out an application form. Fill this in with your company details and requirements and send to your chosen Leightons Opticians branch. You will then be contacted with regards to your account setup. There are no setup costs to join our Corporate Eyecare Scheme.
